Job Description

Responsibilities/ Job Duties: The Intake Coordinator is responsible for accurately entering referrals and verifying eligibility on each referral request received. The Intake Coordinator maintains accurate records in compliance with applicable laws and regulations of the company.

• Enter patient and referral information into tango software system.
• Verify insurance eligibility for each patient.
• Coordinate with the Qualification team as needed.
• Ensures service requests include all required documentation for verifying patient eligibility.
• Call patients and referral sources to obtain any additional information needed related to verifying eligibility.
• Other duties as assigned.
• Demonstrates the following: Ability to multitask in a fast-paced work environment, highly self-motivated, customer service oriented with good communication skills, and detailed.

Minimum Education and Experience Qualification Requirements:

• Must be at least 18 years of age.
• High School Diploma or GED, some college
• High typing speed and accuracy.
• Has basic computer knowledge and experience including Microsoft Word, Excel & typing skills.
• Home care/healthcare background is a plus but not required.
• Must be flexible and willing to work weekends and Holidays.

Schedule/ Shift: M-F, 8am-5pm (Primary) with possible weekend shifts (open SAT & SUN, same hours). **Opportunities for OT!
