Job description

We are hiring for an Infusion RN in Baltimore! This is a great opportunity with a great hospital. This position is outpatient and open to someone with good direct patient care exp.

Description

Provides nursing support for the Cardiac Infusion program. Coordinates patient care activity with Cardiology / Pulmonary on the phone and in the office and clinic, providing necessary support to the physician within established guidelines. Patient care activities within scope of nursing practice will include but are not limited to patient assessment, medication management, collaboration with healthcare providers, patient education, procedural management, and other duties as assigned.

ESSENTIAL FUNCTIONS

  • Provides compassionate and high-quality nursing care by planning, organizing, and directing the nursing functions of patients on the unit.
  • Initiates and implements patient care plan.
  • Makes nursing assignments appropriate to the skill level of employees.
  • Maintains evidence-based standards of patient care.
  • Promotes a culture of patient safety when providing care.
  • Overview

    This role is a Direct Patient Care, managing patients with Cardiology and Pulmology, specifically those dealing with Advanced Heart Failure and other chronic conditions, both related and unrelated to their heart failure. This RN will be responsible for both managing the Cardiac Infusions, but will also be responsible for Care Coordination and Patient Education to help reduce the number of patients in this program that are being readmitted into the hospital.

    Direct Patient Care:

    • Newer program, this will only be around 20%-30% of their day to start, but this will shift over time as they add more patients to the program and continue to expand
    • Administering IV's to patients to support Cardiac Infusions as prescribed by the provider. Different infusions with varying levels of support will be prescribed, RN's must be comfortable working with each.

    Care Coordination:

    • 80% of the job, especially as they add patients
    • Educating patients on their chronic illness, the care plan that's been prescribed, medication reviews, and the importance of following their care plan to avoid serious or fatal events. The goal is reduce the number of patients being admitted back into the hospital
    • Checking in to ensure patients are going to the follow up appointments scheduled
    • Working with Community Health team to get transportation if needed
    • Linking the patient with the necessary social services to provide additional support outside of the clinical setting as needed
