Job Title: In-Home Clinician (Nurse Practitioner or Physician Assistant)

Location:

  • Oregon: Linn, Benton, Marion, Polk, Lincoln counties
  • Wisconsin: Chippewa, Eau Claire, Dane counties

About the Role
We are seeking dedicated and compassionate Nurse Practitioners (NP) or Physician Assistants (PA) to join our team as In-Home Clinicians. In this role, you'll make a meaningful impact by providing personalized, comprehensive in-home care to Medicare patients. You'll collaborate with a multidisciplinary team to improve patient outcomes and quality of life, while also having the opportunity to grow into clinical leadership roles.

Key Responsibilities

  • Conduct in-home patient assessments including medical history reviews, medication reconciliation, and physical exams.
  • Perform evaluations on social determinants of health, mental health, frailty, and other key health factors.
  • Identify and document healthcare needs, gaps in care, and risk factors, creating tailored care plans.
  • Collaborate with primary care providers and specialists to coordinate patient care.
  • Educate patients and caregivers about conditions, medications, and self-management strategies.
  • Partner with social workers, nurses, and other healthcare professionals to ensure holistic care.
  • Maintain compliance with Medicare/Medicaid standards through accurate and timely documentation.

Requirements

  • Active Nurse Practitioner (NP) or Physician Assistant (PA) license in Oregon or Wisconsin.
  • Enrollment with Medicare and the state Medicaid program.
  • Minimum of 3 years of clinical experience in a field-based role, preferably in geriatrics, primary care, or a related field.
  • Familiarity with Medicare Advantage programs and CMS regulations is a plus.
  • Excellent interpersonal and communication skills to engage with patients and their families.
  • Ability to work independently and travel within assigned counties (up to 2 hours driving distance).
  • Bilingual proficiency is a bonus.

About Us
As part of our team, you'll start with 2-3 patient visits per day, each lasting 60-90 minutes, and gradually build to 3-4 visits. Assessments focus on medical history, cognitive evaluations (e.g., clock-drawing tests), functional abilities, and fall risk.
