Hybrid RN Case Manager

This would be a great hybrid opportunity Monday-Friday position for growth and experience! - Very Flexible schedule

Job Description:

• Working with enrolls and families/caretakers to break down any barriers that could come in between them & their treatment plan

• Start off with needs assessment - identify where they are at, assign them acuity level between

• Metrics regarding acuity:
  1= Meet with 1 time a year, call every other month
  2= Meet with 2 times a year, call once a month
  3= Meet with 3 times a year, call twice a month

• Develop a care plan, person centered, work with enroll/caregiver. Work towards goals alongside them

Pay: $40-46/hr (based on experience)

Qualifications:

• 1+ years of Case Management experience (Any Capacity) or Pedaitric Experiance Looking to get Into Case Management

• Registered Nurse

• DC license

• Inpatient/Ambulatory/home health Experience preferred
