Job description

Job Title: Nurse Assessor

Location: travel throughout Bronx or Brooklyn

Schedule: Monday - Friday, 8:00 AM - 5:00 PM

Salary: $50/hr

Overview:

The Nurse Assessor plays a crucial role in evaluating consumers seeking community-based long-term care services and supports. This position involves performing comprehensive assessments, coding and documenting information accurately, and collaborating with interdisciplinary teams to ensure optimal care delivery.

Key Responsibilities:

  • Assessment & Evaluation:
  • Conduct initial assessments of consumers, evaluating health status, functional abilities, strengths, care needs, and preferences.
  • Review consumer medical documentation and health referral forms as needed.
  • Enter and code assessment data into the UAS-NY online application according to the UAS-NY Community Health Assessment Reference Manual.
  • Document outcomes of calls and scheduled appointments.
  • Communication & Collaboration:
  • Enhance communication and collaborative relationships with leadership and interdisciplinary care team members to facilitate service delivery.
  • Review non-qualified assessments with the Clinical Quality Assurance Department.
  • Knowledge & Compliance:
  • Maintain up-to-date knowledge of regulatory and project changes affecting assessment quality and validity.
  • Stay informed about community resources and network services available to the target population.
  • Additional Duties:
  • Perform other duties as assigned by the Regional Nurse Manager or project management.

Assessment Details:

  • Average Assessment Time: Approximately 2.5 hours
  • Remote Assessments: Conduct 3 assessments per day
  • In-Person Assessments: Conduct 2 assessments per day
  • Travel: Includes 1 hour of travel time; mileage is built into the rate
  • Safety: Training provided for security and safety; arrangements for additional personnel if needed for safety concerns

Requirements:

  • Experience: Minimum of 2 years of home health experience within the last 15 years.
  • Licensure: Current Registered Nurse (RN) licensure in New York State.
  • Identification: New York State ID or Driver's License.
  • Technical Skills:
  • Proficient with technology and computer systems.
  • Ability to type at least 27 words per minute.
  • Navigate multiple systems, including Maximus, HDS, and Work Instruction System.
  • Systems Knowledge: Ability to perform online assessments and use various technology platforms effectively.

Preferred Skills:

  • Experience with UAS-NY and related community health assessment systems.
  • Familiarity with HIPAA-compliant virtual platforms (e.g., Zoom).
