Summary: The Housing Stability Specialist is responsible for ensuring the stability and well-being of participants placed in shared housing in the North County region of San Diego. This role mediates conflicts, manages diverse caseloads based on participant acuity, and coordinates with an extensive network of service providers to enhance the long-term success of participants in their new homes based on a Housing First model.
Essential Duties and Responsibilities (other duties may be assigned)
Participant Stabilization in Shared Housing
- Collaborate with each participant to develop and implement individualized housing plans.
- Apply targeted strategies to help participants achieve and maintain stability in shared housing.
- Provide ongoing guidance and resources to support participants in adapting to their new living environments.
Conflict Mediation and Resolution
- Facilitate open communication and mediate conflicts among participants in shared housing to promote a harmonious living environment.
- Develop and implement conflict resolution plans tailored to each unique situation.
- Monitor and address issues or concerns related to shared housing.
Flexible Caseload Management
- Assess participant needs and acuity levels to determine appropriate caseload sizes for individualized support.
- Adjust priorities and resources based on participant circumstances to optimize outcomes.
Coordination with Service Providers and Community Resources
- Collaborate with a diverse network of service providers, such as social service agencies, mental health professionals, employment assistance programs, community organizations, and government entities, to connect participants with additional resources.
- Facilitate seamless access to various resources to maximize support for participants.
Participant Progress Monitoring and Reporting
- Track and document participant progress toward stabilization and self-sufficiency in shared housing placements; maintain accurate and timely case notes in the required database.
- Keep physical and digital files up-to-date to ensure privacy and security compliance (e.g., HIPAA).
- Generate comprehensive reports on participant outcomes to support program improvements and showcase successes.
- Assist program management with contract compliance, audit preparation, records review, and chart assessment.
Teamwork and Development
- Participate in staff meetings, case conferencing, and training as assigned.
- Serve on the events committee, which plans and conducts participant events, meetings, and holiday parties.
- Maintain open communication within and across all departments, providing high-quality customer service to all participants.
- Stay informed on emerging issues and best practices.
Property Owner and Property Management Liaison (as needed)
- Build and maintain strong relationships with property owners, managers, and housing providers to secure and sustain shared housing opportunities.
- Negotiate lease agreements and ensure compliance with fair housing laws and other regulations.
Housing Navigation, Search, and Placement (as needed)
- Conduct thorough searches for suitable shared housing options based on participant needs and preferences.
- Coordinate and facilitate property viewings and support participants with the application and move-in process.
