Medix is now hiring Housing Navigators for a Case Management organization based in the Los Angeles area!
PAY: $25.00 - $30.00/hr
SHIFT: Mon-Fri (8am-3pm); 30hrs/week
OFFICE LOCATION: Los Angeles, CA 90012
REQUIREMENTS:
- Bachelors Degree in Psychology, Human Services, Social Services, Sociology, or any relevant field in addition to 2 years of direct experience with Housing Navigation and Case Management
- GED/High School Diploma with 3-5 years of direct experience in Housing Navigation and Case Management
- Must have a valid driver's license and auto insurance
- Bilingual English/Spanish is a plus
RESPONSIBILITIES AND DUTIES:
- Collaborates with other team members to develop strategies for outreach targeted to consumers who tend to isolate themselves.
- Supports consumers with identifying barriers and finding appropriate healthcare resources,which may include the member's primary care physician, and if applicable, specialty care providers, dental care and vision care providers.
- Assists members with accessing education and employment services that can improve their quality of life.
- Increases awareness of self-help programs and consumer advocacy organizations that promote recovery.
- Participates in off-site presentations that introduce the Cal-AIM program to community partners and potential new consumers.
- Attends staff meetings on a regular basis to provide input towards program development and staff training.
- Provides caregivers or other supportive individuals with information regarding the agency its process and procedures, and the various team members who can assist them.
- Engages in a continuous learning process to learn best practices for engaging consumers.
- Completes intakes and assessments on a timely basis to determine clients housing needs.
- Identifies members who need to complete HMIS CES Surveys.
- Meets with members on a weekly basis to monitor and evaluate housing progression.
- Assists members with developing an individualized housing support plan based upon their housing assessment.
- Identify and secure resources to assist members with obtaining affordable housing and/or subsidizing their rent. This may include evaluating the rental market and networking with potential landlords to build a housing stock.
- Support members with compiling supportive documents and filling out applications pertaining to housing.
- Assist members with applications for supportive and subsidized housing
- Conduct pre-inspections of identified units to ensure they meet Housing Quality Standards (HQS)
- Provide guidance to members that need to request security deposit assistance and manage various fees (ie; utility arrangements and deposits).
- Help members obtain necessary household goods that increase safety and access in the home ( e.g., hospital beds, hoyer lifts, HVAC, and pest control services).
- Facilitate landlord/tenant mediation services to address issues and concerns, and help clients maintain their permanent housing.
- Educate clients on Landlord/Tenants rights.
- Maintain documentation of services provided as required by the program contract and Agency standards. This documentation includes completed service notes, documentation in HMIS and Salesforce, and completion of all required documents in accordance with HUD and funding source.
- Maintain housing charts in accordance with the agency guidelines and other contractual requirements.
- Carry out intervention services that identify potential behaviors that may jeopardize a client's housing, such as: late rental payments, hoarding, substance use, and other violations.
- Collaborate and provide access to community resources that help prevent eviction when clients' housing is potentially jeopardized.
- Assist with benefits advocacy, including assistance with obtaining identification and documentation for SSI eligibility, and support with the SSI application process.
