A large healthcare company is looking for an advanced Health Coordinator / Patient Service Rep to sit onsite 5 days a week in Mullica Hill, NJ! The overall goal of this role is to provide tela-health in a call center setting for patients who have just been dispatched from the hospital. (Note: this is NOT a remote position.) You will help with any and all healthcare needs of the patients and connect them to the correct physicians when needed.

Hours:

• Monday - Friday 8:00 am to 5:00 pm
• No weekends!

Location:

• Mullica Hill, NJ
• 5 days onsite
• No floating to other locations!

Job Description:

• Maintains and monitors a process for addressing customer concerns and preferences
• Acts as an advocate for clients to create a trusting environment for learning, adapting, and healthy change
• Facilitates the care of customers in their homes under the direction of the physician by conducting intake, needs assessment, planning, and reassessment of service needs.
• Acts as a liaison between the attending physician, vendors and the client to ensure non-fragmented, seamless, and holistic care through day-to-day support
• Screens incoming admissions of customers for eligibility for support services and facilitate services from community providers as needed
• Coordinates necessary arrangements and follow up on services for customers in a timely manner
• Responds to customer questions, issues and/or concerns and relay information to the concerned parties.

Requirements:

• At least 3 years experience in healthcare administration
• Strong medical terminology knowledge
