Job description

Job Summary: We are seeking passionate Nurse Practitioners (NPs) to join an innovative, rapidly growing healthcare organization focused on improving care for the senior population. As part of our team, you will provide high-quality primary care in a value-based care model, working within state-of-the-art clinics located in under-served Cleveland communities. This is an exciting opportunity to make a real impact on the lives of seniors and work within a supportive, collaborative environment.

Key Responsibilities:

  • Manage a panel of Medicare-aged patients, providing primary care for both acute and chronic conditions.
  • Develop personalized care plans with a focus on improving patient outcomes and maintaining continuity of care.
  • Conduct comprehensive assessments, including diagnostic exams and health screenings.
  • Prescribe medications and manage patient care with an emphasis on preventive measures.
  • Engage in a team-based approach, collaborating with physicians, nurses, and care teams to provide holistic care.
  • Track and monitor patient progress, ensuring timely follow-ups and addressing social determinants of health.
  • Provide education and guidance to patients and families on managing their health conditions.

Schedule:

  • Monday - Friday: 8:00 AM - 5:00 PM (Last patient at 3:30 PM)
  • 45-minute patient visits, allowing for comprehensive care and relationship-building with patients.

  • Above-market compensation with base salary and performance-based bonus.

Ideal Candidate Qualifications:

  • Active Nurse Practitioner license in Ohio
  • 2+ years of experience in primary care, geriatrics, family medicine, or internal medicine
  • BLS/ACLS certification required
  • Experience with eClinicalWorks or willingness to learn
  • Comfort with a value-based care model and managing a primarily Medicare-aged patient population
  • Strong interpersonal skills and passion for providing patient-centered care

Salary: $125,000 - $165,000 annually, based on experience (Flexible) + Bonus Opportunities
