Job description

Ideal candidates have

-3-5 years experience working with older adults.

-Bilingual in Spanish

-CA Drivers License & Personal transportation

-CRM/EHR experience

-Organized with great clerical skills

-Experience delivering coordinated care for older adults

Expectations for this Role

  • Collaborate with Outreach Specialist and IDT to ensure successful enrollment
  • Conduct home visits to evaluate the prospective participant for potential eligibility for nursing home level of care
  • Conduct home safety evaluations of the prospective participants' home
  • Participate in Enrollment Eligibility meetings to review Level of Care recommendations
  • Complete Nursing Facility Level of Care (NFLOC) assessments and submit to DHCS for review; update/revise the assessment as requested by DHCS
  • Maintain accurate documentation and record all evaluations in CRM software
  • Participate in all PACE Audits by DHCS and CMS
  • Support the Center Director to develop participant retention strategies
  • Maintain knowledge and expertise in eligibility, enrollment, and program specifications for the PACE Program
  • Support business development and census growth by participating and assisting with outreach programs as needed
  • Other duties as assigned

Must Have Skills/Qualifications

  • Passion for delivering amazing coordinated care to frail elders and disabled older adults
  • Associates degree from accredited nursing school; BSN preferred
  • Active Registered Nurses License in the state of California
  • Current CPR certification or able to obtain certification within 30 days of hire
  • 3-5 years of healthcare experience and experience working with frail elderly
  • Require a valid CA driver's license, personal transportation, good driving record and auto insurance as required by State law.
  • Excellent organizational, communications, written and verbal communication skills
  • Excellent documentation skills for CRM/EHR systems
  • Bilingual: English/Spanish preferred
  • Proof of current CPR/BLS certification required or requirement to obtain one within 30 days of being hired
  • Proof of a valid CA driver's license, personal transportation, good driving record and auto insurance as required by State law

Schedule/Shift

M-F 8a-5p normally. Might require occasional evenings and weekends.

  • This role is hybrid, working remotely in the field and onsite at the Los Angeles facility as needed. We anticipate this role will be onsite up to 5 days per month.

Pay Range

$49-60/hour DOE

this position offers a robust compensation package for this role that includes cash compensation and other total rewards. Base pay is based on several factors including but not limited to education, relevant work and industry experience, certifications, and location of the role. Onsite roles include appropriate geographic adjustments, while remote roles are typically priced off national pay data.- Can provide the incentive structure
