Job description

Director of Navigation Services

Shift: M-F 8-5 (with occasional nights - dinner with vendors, etc.)

Location: Need to be able to travel around the Austin area. Travel won't be every day. Home base is flexible.

Pay: $140,000 - $160,000 /annually, with a 10% annual bonus

Start: Early April

Education Requirements: APN, NP, or PA

Experience Requirements:

  • Minimum of 5 years of experience in patient navigation, case management, or a related healthcare role.
  • Needs to have some sort of clinical experience.

Responsibilities:

  • Develop and implement standardized navigation protocols to ensure consistent and high-quality patient care.
  • Collaborate with healthcare providers, including but not limited to urologists, radiation oncologists and pathologists, to coordinate patient care and improve
  • communication.
  • Monitor and evaluate the effectiveness of the navigation program, making data?driven decisions to enhance patient outcomes.
  • Act as a clinical resource to navigator team, providers and clinic staff through provision of patient care and care navigation.
  • Support Patient Advocacy and the Patient's Bill of Rights.
  • Ensure that clinic care standards, including respective pathways, are maintained.
  • Proactively identify and mitigate potential risks to patient safety, including notifying president and respective physician(s) of identified risks and plans to

  • Lead and manage the patient navigation team, providing guidance, support, and professional development opportunities.
  • Conduct weekly one-on-one and team meetings with navigators to gain alignment on all initiatives and expectations.
  • Communicate effectively with leadership on performance, barriers to performance, and expressed team and client needs.
  • Foster a cohesive, responsive team environment with both remote and on-site workers that directly contributes to patient outcomes and Urology Austin's

  • Create and execute monthly business plans to achieve navigator KPIs, exceed internal customer expectations and improve patient outcomes.
  • Coach navigators to achieve targeted goals through maximum resource understanding and utilization.
  • Maintain strong business acumen and critical thinking skills to address patient, physician and navigator needs with minimal delays or disconnects.
  • Develop and manage the navigation program budget, ensuring efficient use of resources.
  • Promote open, effective communication among clinical staff, providers, and ancillary departments.
  • Maintain and utilize provided systems to enhance patient satisfaction and provider productivity.
  • Actively seeks opportunities to improve financial outcomes, engaging staff in the process
  • Create an environment in which staff and peer learning is supported and stimulated.
  • Interpret and support the objectives, policies, and procedures which guide staff and patients.
  • Participate in patient, family, and staff education
  • Serve as a resource liaison for all staff related to the patient's ongoing care to ensure continuity of care as the patient's disease state evolves.
  • Advocate for patients, ensuring they have access to necessary resources, support services, and educational materials.
  • Maintain up-to-date knowledge of urology and oncology treatments, procedures, and best practices.
  • Maintain clinical competency by participating in all required educational updates, meetings, and in-service education.
  • Participate in community outreach and education initiatives to raise awareness about urology services and patient navigation.
