Job description

Job Title: Diabetes Health Educator (bilingual)

Location: Bronx, NY (hybrid, must be open to local travel to office & community events)

Hours: Full time (must be flexible to work community events sometimes on weekends)

Pay: $30/hr (negotiable DOE)

Overview:

This is a year long pilot program in the Bronx for a Spanish bilingual individual who will be going into the community to events to help provide education. Everything will be scripted and there is a chance that they will be doing media coverage (will be prepped and given scripts). If the program is a success (which they expect it to be because of other programs like it) then there is a chance for a conversion to a full time FTE.

Experience:

  • Must be bilingual in Spanish
  • Certificate in Health Education (Certified Health Education Specialists) or BA, Public Health, Health Education or Related Field
  • community health experience
  • Experience working with diverse communities, including faith-based organizations and Local healthcare providers
  • Strong communication, interpersonal and presentation skills
  • Knowledge of health education principles, practices and cultural competency
  • Program planning and evaluation

Primary Responsibilities

  • Assess community health needs and recommend/prioritize educational programs
  • Support in the development and execution of health educational programs
  • Support the creation of objectives and goals
  • Work with an internal team to develop relevant materials and facilitate workshops, training sessions and community events
  • Work with the team lead and project associate to track identified metrics and monitor progress
  • Program effectiveness
  • Work with clinical and care management team to develop culturally sensitive and competent educational materials and resources
  • Work to promote healthy behaviors in the community (based on area of focus)
