Dementia Care Nurse Navigator

Job description

In this role, you will provide essential support to patients with dementia and their families, helping them navigate the complexities of the dementia diagnosis, treatment, available resources, education, crisis prevention, and care coordination.

Schedule: Hybrid work schedule: 2 days remote, 3 days on-site at the facility.
- Occasional evenings or weekends may be required for patient or caregiver support.
- The position operates primarily during normal business hours, Monday through Friday. Flexibility in hours and location may be possible.

Location: 6 different facilities within Baltimore, you will just be traveling to 1 each in person work day.

Start Date: ASAP

Duration: 4 month contract - most likely will extend!

Qualifications:
- Registered Nurse (RN) with a valid and active license.
- Experience in dementia care, gerontology, or a related field is highly preferred.
- Knowledge of clinical processes, medical management, and community resources for aging populations.
- Strong communication and interpersonal skills to educate and support both patients and caregivers.
- Proficiency in using Electronic Health Records (EHR) systems, such as Epic.
- Ability to work effectively in a hybrid environment (2 days remote, 3 days at the facility).

Key Responsibilities:
Clinical Evaluation:

- Assess patient health through histories and evaluations.
- Develop care plans, including medical, behavioral, and community resources.
- Implement effective communication with dementia patients and caregivers.
- Facilitate clinical referrals and ensure smooth processing.
Care Coordination & Monitoring:
- Review and prepare for upcoming patient appointments (e.g., Epic, Telehealth).
- Ensure tests, referrals, and required services are completed.
- Collaborate with providers on test results and care plans.
- Coordinate with referring physicians for patient needs.
- Educate patients and families on dementia care and coping strategies.
Communication & Collaboration:
- Act as the primary contact for GUIDE patients and caregivers.
- Serve as a liaison between the GUIDE team and referring physicians.
- Provide emotional support and counseling.
- Work with external case management and social services.
- Assist in developing patient and caregiver education programs.
