Job description

Medix is now hiring a Community Health Worker Registered Nurse for a growing healthcare organization!

Shift: Mon-Fri (8am-5pm), training is 3 weeks

Requirements:

  • Must reside in the Chicago Metropolitan area
  • Valid IL RN License
  • Bachelors Degree in Nursing (BSN)
  • Current driver's license, reliable transportation, car insurance, and an acceptable driving record with willingness to travel within approximately 50 miles to meet in person with patients in their homes or other medical facilities including hospitals, and local physician offices.
  • 2+ years of experience with adult medicine and pediatric patients in primary care/ambulatory care, home health agency, skilled nursing facility, or hospital medical-surgical setting, within the past five years
  • Strong clinical assessment, physical exam, and critical thinking skills. Comfortable discussing a wide variety of medical conditions.

Job Duties:

  • Conduct in-person visits to assess patient health, educational, and psychosocial needs in homes, hospitals, or during medical appointments.
  • Develop comprehensive, individualized care plans with patients and a multidisciplinary team to achieve desired goals.
  • Provide education on managing chronic conditions, medications, and lifestyle changes to patients and families.
  • Identify and address barriers to care, including social determinants of health, and connect patients to community resources with the help of a remote social worker and community health worker.
  • Collaborate with healthcare teams, family members, and community physicians for coordinated care.
  • Monitor and adjust care plans based on patient response, ensuring early intervention for high-risk patients.
  • Maintain documentation for all care management activities and participate in care coordination meetings.
  • Engage with local providers and hospitals for outreach and member engagement.
  • Stay updated on best practices in chronic disease management and provide compassionate follow-up care.
  • Support patients and families during acute healthcare episodes with coordinated case management.
