Job description

Job Overview:

The Community Care Coordinator is a key part of our Street Medicine team, helping people experiencing homelessness get the healthcare and support they need. You will build trust with individuals, assist with registration, and connect them to important services like housing, food, and medical care. This role involves traveling to outreach locations, working closely with the healthcare team, and providing case management support to clients.



Key Responsibilities:

  • Verify eligibility & register clients: Check that clients qualify for services and complete all required forms. Register clients into the EPIC EMR system during outreach.

  • Engage with clients in the community: Be the first point of contact for people in shelters, encampments, and other community areas, building trust and encouraging them to get care.

  • Provide outreach support: Travel to shelters and encampments to find clients who need help. Assist with transportation and set up for outreach activities.

  • Coordinate care: Work with healthcare providers to create care plans, schedule follow-ups, and connect clients to services like housing and food assistance.

  • Case management: Help clients by scheduling appointments, tracking referrals, and following up to make sure their needs are met.

  • Build trust: Use your personal experience and cultural understanding to connect with clients and help them feel comfortable engaging with the program.

  • Keep records: Document client interactions and services, following program guidelines and ensuring privacy standards are met.

  • Support team safety: Assist in keeping the outreach team safe and help manage emergency situations when needed.

Schedule: Monday - Friday 8am - 5pm
