Job description

Pay:

  • Pay: $21.05 - $23.30/hr

Schedule:

  • Sun-Thurs 11AM-7PM
  • Tues-Sat 8AM-4PM
  • Hours and days can be flexible!

Day Center Locations: Denver, Aurora and Lakewood (home visits will be within a 10-20 mile radius of the day centers, and mileage reimbursement is offered!)

Role:

  • This CNA will be completing home visits for participants in their PACE program for their Denver location
  • Each CNA will be given an InnovAge iPhone - There is an App installed that will give them a clear layout of their daily schedules
  • Charting is minimal, they use the iPhone to check off bubbles once the tasks that were listed are completed
  • Will be directly following the plan of care to assist participants with activities of daily living
  • Tasks can include - Dressing, grooming, hair care, mouth care, eating, minor housekeeping (tidying up to ensure that participants are comfortable in their area), medication reminders (not passing meds), helping with small things like putting groceries away, minor meal prep (not cooking, but maybe helping with cereal or microwaveable items)
  • Will record vital signs when necessary
  • Report any changes to health status to RN that may be observed during visit (skin, weight, appetite, etc)
  • 7-10 patients/day, typically about 30min-1 hr appointments, when Laundry is included then visits may be about 2 hours.
