Job description

Job Summary:

Medix is hiring a dedicated and compassionate Licensed Independent Clinical Social Worker (LICSW) for a well known Pediatric Rehabilitation and Specialized Care Facility in Washington, DC.

This role is vital in supporting medically complex, technology-dependent pediatric patients and their families as they navigate post-acute care. The Social Worker will play a key role in interdisciplinary rounds, initial assessments, complex discharge planning, and connecting families with essential resources.

This position requires experience in case management rather than therapy and will involve working closely with families, healthcare providers, legal teams, and community services to ensure holistic support for patients.

Schedule & Caseload:

  • Full-time: Monday - Friday, 8:30 AM - 5:00 PM
  • On-call: Rotating weekends (once a month, telephonic)
  • Caseload: Currently 18 patients, increasing to 26 over time
  • All pediatric facility patients will be seen by a Social Worker, with those facing significant healthcare barriers receiving more frequent support

Key Responsibilities:

Patient & Family Support:

  • Conduct comprehensive psycho-social assessments upon admission, addressing legal, family, physical, and mental health factors.
  • Arrange family meetings within 72 hours of patient admission, ensuring collaboration with the care team.
  • Serve as the primary point of contact for families, fostering trust and comfort while addressing concerns about patient care.
  • Provide education and guidance on local resources, Medicaid services, and community programs to assist families in overcoming healthcare barriers.
  • Work closely with interpreter services to ensure clear communication with non-English-speaking families.
  • Support families facing educational challenges in understanding their child's medical and support needs.
  • Manage visitor policies and logistics for patients, ensuring compliance with facility regulations.

Interdisciplinary Collaboration:

  • Participate in daily rounds with a multidisciplinary team, including physicians, nurses, respiratory therapists, RN Case Managers, and therapists.
  • Work closely with RN Case Managers to develop individualized care plans.
  • Coordinate with children's attorneys and legal teams to address legal issues impacting patients.

Discharge Planning & Resource Coordination:

  • Lead complex discharge planning for patients requiring extended post-acute stays.
  • Assist families with housing, food access, government support, child care, and other essential services as needed.
  • Arrange post-discharge follow-up care and appointments to ensure continuity of care.
  • Advocate for families in securing Medicaid benefits and other financial assistance programs.

Long-Term & Specialized Support:

  • Work with families of chronically ill children who will remain in the facility long-term, helping them navigate financial and daily living challenges.
  • Provide ongoing support to patients transitioning through rehabilitation and non-acute medical care.

Qualifications & Requirements:

  • LICSW required (or equivalent state licensure)
  • Experience in case management (not therapy-focused roles)
  • Strong knowledge of Medicaid resources and community-based social services
  • Experience working with medically complex pediatric patients preferred
  • Ability to work in a fast-paced, interdisciplinary healthcare setting
  • Excellent communication and advocacy skills, with the ability to support families facing significant social and financial barriers

Training & Onboarding

  • New hires will shadow an experienced Social Worker to gain a comprehensive understanding of the role and facility operations.
