Location: Washington, D.C.

Job Summary: We are seeking a compassionate and dedicated Clinical Social Worker to join our esteemed healthcare team. As a Clinical Social Worker, you will play a crucial role in providing direct social work services to our patients and their families, ensuring they receive the best care and support possible. This role is essential for promoting positive health outcomes and fostering a supportive community environment.

Hours: M-F 8-5

Key Responsibilities:

  • Direct Patient Care: Provide clinical social work services to patients and families, including individual and group counseling on the impact of illness and hospitalization.
  • Patient Advocacy: Champion patient rights and ensure confidentiality, advocating for patient needs within the healthcare system.
  • Treatment Planning: Assess patient and family needs to develop and implement effective treatment plans, adjusting as necessary based on patient progress.
  • Interdisciplinary Collaboration: Work closely with other healthcare professionals to ensure comprehensive and continuous care for patients.
  • Resource Education: Educate patients and families about available community resources and services to support their healthcare journey.
  • Communication Facilitation: Serve as a liaison between the patient, family, and healthcare team, facilitating clear and effective communication.
  • Record Maintenance: Maintain detailed and accurate records of patient interactions and treatments.
  • Team Participation: Actively participate in team meetings and case conferences to coordinate care and discuss patient progress.

Qualifications:

  • Education: Master's degree in Social Work from an accredited institution.
  • Experience: At least 1-2 years of clinical social work experience, preferably with acute and chronically ill patients in a hospital setting.
  • Licensure: Must be licensed or license-eligible as an Independent Clinical Social Worker (LICSW) or LGSW in Washington, D.C.
  • Skills: Strong commitment to patient advocacy, excellent communication and interpersonal skills, and a thorough understanding of the healthcare system.
