Job description

This role will involve providing expert medication therapy consultation to patients and healthcare providers, collaborating with the Engineering team and leadership to improve software platforms, and supervising the Clinical Care support team. The Clinical Pharmacist will have a direct impact on patient care and the delivery of core clinical services, keeping up-to-date on advancements in the fast-evolving field of pharmaceuticals, as well as emerging treatment guidelines and best practices.

What You'll be Doing:

  • Evaluate, assess, and document patient data in the software platform and other programs, preparing written reports and providing oral reports on patient care and team progress.
  • Complete thorough and efficient medication reviews and/or adherence counseling appointments.
  • Review medication reviews and counseling done by other virtual pharmacist users, as assigned.
  • Staff various clinical shifts and collaborate with automation and new technologies.
  • Respond to inquiries from patients, providers, and team members.
  • Participate in process improvement meetings to enhance workflow efficiency.
  • Communicate effectively in written and oral formats and provide daily sign-out when necessary.
  • Follow up with clinics, providers, pharmacies, and patients to resolve concerns in a timely manner.
  • Develop strategies to optimize medication utilization and meet quality measures and other established goals.
  • Identify opportunities for improvement in workflow processes and software platforms.
  • Assist with obtaining referrals and authorizations.
  • Guide and oversee other Clinical Care team members.
  • Assist in orienting new Virtual Pharmacist users to workflows.
  • Evaluate and help develop clinical algorithms.
  • Adhere to company policies and all relevant state and federal rules and regulations.

Our Ideal Candidate Possesses:

  • Doctor of Pharmacy (PharmD) from an accredited college/university.
  • Licensed Pharmacist in multiple states and in good standing (California RPh license required).
  • Completion of a PGY-1 residency, or equivalent experience.
  • Ability to assess the appropriateness of prescriptions considering age, disease states, and other relevant factors.
  • Strong patient engagement skills, with a focus on promoting adherence with empathy and compassion.
  • Knowledge of chronic disease states like diabetes, heart failure, COPD, asthma, cardiovascular disease, hypertension, dyslipidemia, anxiety, depression, bipolar disorders, and thought disorders.
  • Strong decision-making skills and ability to work independently to meet deadlines.
  • Experience working with diverse patient populations from varying cultural, religious, and socio-economic backgrounds.
  • In-depth knowledge of how factors like age, sex, co-existing conditions, concurrent drug therapy, food, and diagnostic procedures affect drug activity.
  • Familiarity with regulatory requirements at the local, state, and federal levels.
  • In-depth understanding of prescription and over-the-counter medications, including indications, efficacy, dosage, mechanisms of action, contraindications, and potential side effects.

Nice-to-Haves:

  • Experience with electronic medical records (EMRs).
  • Board certification in a specialty area.
  • Experience in a busy healthcare environment.
  • Bilingual fluency in both English and Spanish.
