We are looking for an Clinical Assessment Nurse (RN)(UAS) on a Hybrid basis in the Bronx of NY!

We are looking for RNs in NY that are interested in the administrative side or nursing with great time management documentation skills!

Overview:

• The Nurse will work within an MLTC program to guide a prospective participant through the clinical process from pre-enrollment to start of care; and to be a resource to the Interdisciplinary Team (IDT).
• This role in administrative, non-direct patient care. No UAS experience is required, but the candidate must be willing to do 2-3 days of field work.

Specifics:

Requirements: Active RN NY license (1 year of nursing experience)

Schedule: Monday-Friday 8:30am-5:30pm (Full time only)

Pay: $99,000 - $108,000 (depending on experience)

Setting/Work Conditions: Mix of Remote and Field based work

Borough: Bronx NY

Hybrid basis: 2-3 days remote, 2-3 days field (depending on schedule)

Job Summary:

• Conducts telephonic and virtual assessments for prospective members and re-assessments of current members using the Uniform Assessment System (UAS)
• Makes enrollment and continued eligibility determinations by utilizing clinical expertise and critical thinking skills.
• Provides community-based education for our frail population including self-care techniques and preventative care strategies
• Ensures (prospective) members and significant others/responsible parties understand and agree with enrollment into the managed long-term care plan

Note:

• 4 week training provided
• This role in flexible, but require 2-3 visits per day
• Laptop and any equipment needed for the position will be provided
• Mileage is reimbursed
