Certified Nursing Assistant

Job description

Pay: $23-$26.50

Role:

  • This CNA will be completing home visits for particpants in their PACE program for their Denver location
  • Each CNA will be given an iPhone - There is an App installed that will give them a clear layout of their daily schedules (what particpants they need to see, and what tasks need to be done during the visit)
  • Charting is minimal, they use the iPhone to check off bubbles once the tasks that were listed are completed
  • Will be directly following the plan of care to assist particpants with activies of daily living
  • Tasks can include - Dressing, grooming, hair care, mouth care, eating, minor housekeeping (tighting up to the particpants are comfortable in their area), medication reminders (not passing meds), helping with small things like putting groceries away, minor meal prep (not cooking, but maybe helping with cereal or microwaveable items)
  • Will record vital signs when neccessary
  • Report any changes to health status to RN that may be observed during visit (skin, weight, appetite, etc)
  • 7-10 patients/day, typically about 30min-1 hr appointments, when Laundry is included then visits may be about 2 hours
  • 60 day reviews will be performed by the RN during an in-home visit, going over care/treatment plan and evaluating for any neccessary changes
  • Once training is completed (the first week), they will not have any requirements to report to the Day Center. CNA's will be able to start/end their days at home (unless they need to go to the Day Center to pick up supplies)
  • Home visits will all be within a 10-20 mile radius of the Denver Day Center - Will be reimbursed 65.5 cents/mile
  • Looking for at least 1 year of experience working with the elderly/frail *Homecare, assisted living, hospice, short term / long term care facilities, etc. (They are open to candidates that have under a year as they have the support to help train, just need to be the right fit!)

Schedule:

  • Sun-Thurs or Tues-Sat 7AM-330PM
  • 8 hour shifts, can be flexible w hours as needed
3-5 Must Have Skills/Qualifications:
  • Active CNA with Colorado
  • Drivers License/auto insurance
  • Minimum of one year experience working the frail or elderly
