Job Title: Case Manager - Intensive Case Management Program

Description: Medix is looking for outgoing, passionate, and driven individuals who are wanting to give back to their community! We are hiring for an Intensive Case Management position with a growing organization. The ICMS team is dedicated to supporting formerly homeless individuals with chronic medical conditions and co-occurring disorders. As a Case Manager, you'll collaborate with partners and property management to provide essential services aimed at enhancing residents' self-sufficiency and housing stability.

Responsibilities:

  • Work closely with referring agencies like Coordinated Entry System (CES) and DHS.
  • Assist with lease-up paperwork processes.
  • Establish connections to mental health, substance abuse, employment, and other needed services.
  • Provide strength-based case management and service coordination.
  • Attend appointments with clients as needed.
  • Maintain accurate documentation and submit required reports on time.
  • Provide crisis management services and interventions when necessary.

Requirements:

  • Bachelor's degree in Social Work or related field preferred.
  • Minimum four years' experience working with homeless individuals or vulnerable populations.
  • Strong oral and written communication skills.
  • Ability to work independently and as part of a team.
  • Flexibility in scheduling and prioritizing tasks.
  • Comfortable working with diverse communities.
  • Experience in crisis settings and emergency response preferred.

Physical Demands/Working Conditions:

  • Work in both office and community settings.
  • Ability to travel within the region as needed.
  • Physically capable of standing, walking, lifting up to 10 pounds, and other physical activities.

Pay Range:

  • Preferred hiring range: $23.00 to $26.00
