Case Manager
Job description
Large nonprofit organization serving southern California is seeking a Case Manager to support the housing services. This person will be working in the West LA area supporting clients in all cycles of homelessness.
Pay: $22-26/hr
Location: Office located in East Hollywood area
Territory is a 10 mile radius from there
Schedule: Mon-Fri, 8am-4:30p (can be flexible)
Some duties include:
Conducts intake and individualized needs assessment for all clients and collaborates to develop Individualized
Service Plan (ISP) that addresses housing goals, maintaining/increasing income and other personal goals identified
by the client including medical, mental health, substance use, financial resources, vocational, and social support
needs.
Must have:
- Two (2) years of experience in healthcare, case management, homeless services or working with vulnerable
populations or a related field.
- Own vehicle and a valid CA Driver license.
