Position Title: Case Manager
Location: Bronx, NY
Job Type: Full-Time, Monday to Friday, 9:00 AM - 5:00 PM
Salary Range: $27.81 - $32.92 per hour, depending on experience

Principal Mission:
The Case Manager provides patient-centered case management services to an assigned panel of patients to support access to care and achieve improved health outcomes. The Case Manager will utilize strong organizational skills to complete assessments, develop patient-centered plans of care, coordinate resources, and conduct outreach. This role focuses on promoting a safe, professional healthcare environment and ensuring all services are consistent with the organization's mission to drive patient care excellence.

Responsibilities Include:

  • Promote a safe, cooperative, and professional healthcare environment to ensure optimal patient care.
  • Provide outreach, intake, comprehensive assessments, and ongoing reassessments for assigned patients.
  • Develop a patient-centered Plan of Care (POC) based on assessments to address health and social needs.
  • Manage a caseload of approximately 25 patients, including conducting face-to-face meetings and minimal home visits (up to 2 per month).
  • Participate in case conferences and work closely with a multidisciplinary team to coordinate care.
  • Assist patients in securing medical and social service appointments and follow up on missed appointments.
  • Accurately document all services provided in the electronic health record.
  • Conduct ongoing education to help patients adhere to medical appointments and treatment plans.
  • Actively participate in trainings to stay current with healthcare regulations and best practices.
  • Respond to emergency situations such as inpatient admissions or emergency department visits and provide necessary interventions.
  • Utilize electronic systems to track participant activities and progress.

Qualifications:

  • Education/Experience Options:
    • Master's degree in a qualifying health or human service field with 1 year of experience
    • Bachelor's degree in a qualifying health or human service field with 2 years of experience
    • Bachelor's degree in any field with 3 years of experience as a Care Manager working with individuals with HIV/AIDS, Serious Mental Illness, Substance Use Disorders, or individuals transitioning from institutional care
    • Credentialed Alcoholism and Substance Abuse Counselor (CASAC) with 2 years of Care Management experience
  • Preferred: A minimum of 2 years of care management experience
  • Skills: Excellent communication skills, verbal and written. Working knowledge of physical and/or mental health. Proficiency in electronic data collection systems
  • Language Proficiency: Bilingual and able to read, write, and speak Spanish proficiently is preferred but not required
  • Travel: Ability to travel locally within the Bronx to meet with patients and attend site/home visits as needed
