Job description

  1. Responsibilities/ Job Duties:
    • Prioritize customer service and communicate effectively with patients, visitors, and co-workers.
    • Uphold standards for patient-centered care and excellence.
    • Perform outreach, intake, and assessments (including HARP and others).
    • Develop and update patient-centered care plans based on assessments.
    • Manage a panel of patients, coordinating care and addressing health/social needs.
    • Participate in case conferences with multidisciplinary teams.
    • Assess eligibility for services and ensure access to entitlements and benefits.
    • Create service plans and resolve barriers to care..
    • Monitor patients' use of services (appointments, treatments) through direct contact.
    • Document all services in electronic health records.
    • Identify and communicate health and psychosocial concerns to relevant staff.
    • Respond to hospital admissions or emergencies with necessary interventions.
    • Maintain caseload tracking to manage care plans effectively.
    • Assist patients with medical/social appointments and follow-ups.
    • Educate patients on adherence to treatment plans and maintaining health.
    • Conduct site and home visits as needed.
    • Attend training for skill development and healthcare updates.
    • Connect patients with appropriate resources and programs.
    • Uphold a case load of around 25.
  2. Minimum Education and Experience Qualification Requirements:
    • A minimum of two (2) years experience in case management required.
    • Prior experience in the Health Home model preferred.
    • Excellent communication skills, verbal and written
    • Working knowledge of physical and/or mental health
    • Ability to travel to meet with patients
    • Proficiency in electronic data collection systems
    • Bilingual Spanish/English preferred
  3. Schedule/ Shift:
    • Monday - Friday 9am-5pm
  4. Benefits:
