Job description

Overview: Join our dedicated team and make a meaningful impact in our community! This role involves conducting comprehensive client assessments and collaborating on tailored service plans to support housing, financial stability, and personal goals. We're looking for someone passionate about providing holistic support and maintaining high standards of documentation and client confidentiality.

Core Responsibilities:

  • Client Assessment & Service Planning:

    • Conduct thorough intake assessments and develop Individualized Service Plans (ISP) to address clients' housing, income, and personal goals, including medical, mental health, substance use, financial, vocational, and social support needs.
    • Provide referrals, linkages, and support to resources to help clients achieve their ISP goals.
  • Program Management:

    • Complete program-specific assessments (e.g., VI-SPDAT) and evaluate client strengths and challenges.
    • Conduct 90-day reviews with clients to assess progress and adjust goals as needed.
  • Documentation & Confidentiality:

    • Maintain accurate and up-to-date client files, including timely case notes, in compliance with Program and PATH policies.
    • Ensure confidentiality of client information in accordance with HIPAA and other relevant guidelines.
  • Client Support & Crisis Intervention:

    • Provide crisis intervention and risk assessment services to enhance client safety and problem-solving skills.
    • Utilize motivational interviewing and trauma-informed care approaches in service delivery.
  • Resource Management:

    • Stay informed about community resources and integrate them into service delivery for comprehensive client support.

    • Demonstrate a commitment to diversity, equity, and inclusion through service delivery, continuous learning, and fostering an inclusive work environment.

    • Perform other duties as assigned or required.

Required Qualifications:

  • Mandatory Skills/Qualifications:

    • All levels of education and experience are welcome.
    • Employment Eligibility Verification.
    • Perform work in California.
    • Successful completion of background screening and drug test.
    • Updated Tuberculosis Test.
  • Preferred Qualifications:

    • Two (2) years of experience in healthcare, case management, homeless services, or working with vulnerable populations, or a related field.
