Job description

Case Manager for Veteran Services

Position Title: Case Manager (Remote, 1 day per month office requirement)

Schedule: Monday - Friday, 9:00 AM - 5:00 PM

About the Role: Join our dedicated team as a Case Manager and play a crucial role in supporting veterans within the community. You will be responsible for coordinating and monitoring services to enhance the quality of life and promote independence among veterans. Your work will involve assessing needs, identifying support options, and collaborating with a multidisciplinary team to achieve the best outcomes for those you serve.

Key Responsibilities:

  • Conduct comprehensive in-home assessments and prepare detailed reports to capture clients' functional needs and provide service recommendations.
  • Research and identify appropriate services, support networks, and funding options to address assessment recommendations.
  • Collaborate with a multidisciplinary team for ongoing evaluation of needs, services, and SMART goals.
  • Review provider invoices for accuracy, ensuring alignment with contractual agreements, and assess progress toward client goals.
  • Maintain regular communication with clients, community providers, and funding sources.
  • Keep accurate and current documentation, including case notes, monthly summaries, and client files.
  • Participate in team meetings, trainings, and in-services as requested.
  • Maintain precise billing records for all clinical and administrative activities.

Qualifications:

  • Bachelor's degree in a health care or related field (e.g., rehabilitation counseling, social work, speech and language therapy, occupational therapy, psychology, education, veterans' services).
  • Minimum of two years of case management experience, including managing clients with disabilities.

Preferred Skills and Qualifications:

  • Excellent oral and written communication and problem-solving skills.
  • Ability to be flexible and work effectively as part of a team.
  • Experience working with veterans and with catastrophic injuries such as brain injuries, blast injuries, spinal cord injuries, and other neurological conditions.
  • Bilingual skills and Brain Injury Specialist Certification are a plus.
