Job description

Medix is looking to hire an Case Coordinator to join a residential facility with a community health organization in Los Angeles, CA 90027. The Case Coordinator will work in collaboration with the mental health team, which consists of the Medical Director and Licensed Clinical Social Worker (LCSW). They are responsible for delivering accurate information about the company's programs and community social services to patient seeking services. The Case coordinator is also responsible for providing short-term service plan for the referred patients and integrate mental health programs and treatments for patients as priority.

Responsibilities:

  • Appointment scheduling and reminder calls
  • Psychological Testing- PHQ9/GAD 7
  • Facilitation of referrals

Qualifications:

  • Bachelor's degree in social work or related field or a minimum of 2 years in patient/client services
  • Bilingual in English AND API language OR Spanish
  • Ability to be proactive with tasks
  • Ability to communicate
  • EMR experience preferred but not required

Schedule:

Monday - Friday between 8am - 5pm - HYBRID ROLE

Pay rate: $20-$22/hr
