Job Description:
Responsibilities/ Job Duties:
- Under the direct supervision of the Director, Health Services, the Care Coordinator will be responsible for assisting members with accessing and optimizing the use of resources to address social determinants of health, performing assessments, and assisting members to navigate through the health care ecosystem.
- Conduct Health Risk Assessments
- Establish effective relationships with individuals while promoting general support.
- Assist members with value-add benefits such as transportation, dental, vision programs.
- Contact PCP offices for member information as needed.
- Send member/provider notifications for Interdisciplinary Team meetings.
- Provide education and referral assistance regarding energy assistance programs, housing assistance programs, and other social determinants of health with the goal of promoting self-sufficiency and improved health care outcomes.
- Provide assistance with linking to community resources.
- Assist members with referrals to Case Management or Disease Management, if desired.
- Assist members with arranging post-discharge meals, transportation, and other services offered b
- Participate in staff meetings, training sessions, interdisciplinary care team meetings, as directed.
- Document activities in Care Management Systems and tools for tracking.
- Perform telephonic, written, and/or electronic follow-up with members, as required.
- Maintain confidentiality of member's health information in accordance with HIPAA
3-5 Must Have Skills/Qualifications
- PC proficiency to include Word, Excel, PowerPoint, database experience, and Web-based applications.
- Verbal and written communication skills including listening, discussing, and documenting needs with members, providers, internal staff/management, external vendors, and community resources.
- Previous experience with health education, human services, and supporting behavioral change; experience w Medicare Part D preferred
- Associate or bachelor's Degree in the Human Services and/or Health-related field is required, or an equivalent combination of education and experience from which comparable knowledge, skills, and abilities can be acquired. customer service skills; experience conducting health risk assessments and outbound calls
