Job description

Are you looking to coordinate care for those with chronic illnesses? Step away from direct patient care?

We are hiring a Care Coordination (Registered Nurse, RN) in New York, 11101. This is a great way to kick start a career is Care Management and educate those on chronic disease.

This role in an outpatient health center, please note this is an onsite role!


Duties:

The Care Coordinator will be going through reports of chronic care coordinators and meeting with patients in office or via telephone. The focus will be to follow up with patients who have been to the emergency room and how we can keep them home moving forward.

  • Anticipates patient needs and ensures that pre-visit planning is completed before patient visit
  • Prepares, distributes, and analyzes daily patient rosters
  • Coordinates and documents daily patient care conferences
  • Provides proactive outreach to high risk patients
  • Facilitates advanced management of care to an identified high risk client population
  • Develops care plans specific to individual patients by collaborating with providers
  • Assists patients in setting treatment goals and assesses potential barriers
  • Educates patients on individual care plans

Specifics:

Requirements: RN License (outpatient experience, chronic care experience; Spanish bilingual is highly preferred)

Hours: Monday - Friday 9am-5pm (no part time options)

Pay: $46-52/hr

Location: New york, New York 11101 (onsite only)
