Job Description

The Nurse Care Manager plays a key role in supporting members and their families to maintain the most independent living situation possible. The Nurse Care Manager will ensure the delivery of consistent care across the healthcare continuum by assessing and monitoring the member's health needs and status. This role involves authorizing covered services, coordinating care, and collaborating with primary care practitioners, interdisciplinary teams, and members/families to achieve optimal health outcomes.


Responsibilities/ Job Duties:

  • Collaborate with members and their families to assess and manage healthcare needs, ensuring an independent living situation.
  • Coordinate and authorize covered services for members, regardless of payer.
  • Communicate effectively with members, family, caregivers, primary care practitioners, and interdisciplinary teams to ensure consistent care.
  • Monitor and assess member status, adjusting care plans as necessary to ensure optimal health outcomes.
  • Work closely with the healthcare team to facilitate efficient and timely care delivery across the continuum.
  • Maintain accurate documentation of care coordination, authorizations, and member interactions.

Minimum Education and Experience Qualification Requirements:

  • Current NYS RN License and registration required.
  • Bachelor of Science in Nursing required.
  • A minimum of three (3) years of RN experience.
  • Clinical expertise in geriatrics is preferred.
  • Managed long-term care experience is preferred.
  • Bilingual in Cantonese, Mandarin, Russian or Spanish is required.
