Job description

Position Summary:

We are seeking a Bilingual (Russian) Case Manager to play a central role in coordinating care and services for high-utilization Medicaid members with chronic medical and behavioral health conditions. Responsibilities include conducting assessments, developing care plans, and ensuring timely documentation. You will also assist with program eligibility, care plan assessments, and acuity reassessments.

Essential Duties and Responsibilities:

  • Obtain required Care Management enrollment consent.
  • Conduct needs assessments to determine appropriate care management level.
  • Oversee development of individualized care plans.
  • Ensure accurate documentation using Care Management Portals and Electronic Health Records (EHRs).
  • Serve as an advocate for members.
  • Promote wellness and prevention through resource and service connections.
  • Educate caregivers on chronic condition care and preventive interventions.
  • Assist members in obtaining and maintaining public benefits.
  • Communicate effectively with members and their families.
  • Conduct care planning meetings and coordinate services.
  • Identify and manage community-based resources and referrals.
  • Participate in discharge planning for hospital admissions.
  • Attend ongoing staff development training.

Qualifications:

  • Bachelor's degree in a related field.
  • Minimum 2 years of relevant experience.
  • Excellent interpersonal and organizational skills.
  • Ability to engage individuals and diffuse difficult situations.
  • Health Home experience or equivalent program preferred.
  • Multidisciplinary and field-based clinical work experience preferred.
  • Strong written and verbal communication skills.
  • Bilingual skills in Russian strongly preferred.
