Job description

Medix is looking to hire a Behavioral Health Navigator for a non-profit organization in Aurora, Colorado.

Responsibilities:

  • This position is responsible for the direct service and care coordination of enrolled participants of the program.
  • Conduct psychosocial evaluations to assess substance use, mental health, and trauma-related needs.
  • Provide individual counseling sessions focused on substance use recovery, harm reduction, and relapse prevention.
  • Facilitate group education and support sessions to build recovery skills and foster peer support.
  • Evaluate eligibility for community programs and coordinate client referrals to treatment and recovery support services.
  • Maintain accurate and confidential documentation in compliance with HIPAA and agency policies.
  • Educate and collaborate with care management teams to ensure continuity of care.
  • Develop and maintain relationships with inpatient and outpatient treatment providers to improve access to care.
  • Conduct data analysis and reporting on substance use trends, treatment effectiveness, and client outcomes.
  • Deliver culturally responsive interventions that address the stages of change and encourage active participation in recovery.
  • Identify risk management concerns and work with a supervisor to develop ethical and legal solutions.
  • Participate in continuing education to stay updated on evidence-based treatment approaches.
  • Ensure timely and accurate data entry in appropriate databases.
  • Attend case staffing meetings with community partners, stakeholders, and the care team to discuss treatment progress and goals.
  • Perform other relevant duties as assigned.

Requirements / Experience:

  • Master's degree in social work, Clinical Psychology, Counseling, or related field.
  • LCSW, LPC, LMFT, LAC, or another relevant clinical licensure in good standing in the state of Colorado or registered as a licensure candidate.

Preferred Qualifications:

  • 2+ years of experience in crisis interventions and evaluations working with individuals with complex mental concerns (preferred).
  • Previous experience working with the re-entry population (preferred).

Schedule:

  • Monday -Friday 8:00am-5:00 pm, rotating Saturday 8:00am-12:00pm

Pay range:

$60,0000-70,000 annual salary (BOE)
