Salary Expectations -$125K-$170k (depending on experience)

Work Location: Tulsa, OK

Shift: Monday through Friday between 8am-5pm

General Overview:

We are seeking dedicated and compassionate Nurse Practitioners (NP's) to join our Healthcare Team. As a Nurse Practitioner (NP) you will support a new, innovative healthcare provider devoted to improving the lives of our senior members. Providers will be responsible for Primary Care services to patients, not by the volume but by providing high quality care. Nurse Practitioners (NP's) will have a small pane size that allows Providers to focus on building relationships with their patients.

Essential Functions:

  • Perform comprehensive health assessments for senior patients, including history-taking, physical examinations, and diagnostic testing.
  • Diagnose and treat common acute and chronic conditions, including hypertension, diabetes, COPD, arthritis, heart disease, and more.
  • Develop and implement individualized care plans to manage chronic diseases and prevent hospitalizations.
  • Provide patient education on disease management, medication adherence, lifestyle changes, and preventive health measures.
  • Collaborate with physicians, specialists, and other healthcare providers to coordinate care and ensure holistic treatment plans.
  • Prescribe medications and therapies as needed, and monitor patient responses to treatment.
  • Conduct annual wellness exams and preventive screenings in accordance with age-appropriate guidelines.
  • Monitor and manage the medication regimen for patients, ensuring safe and effective use of prescribed therapies.
  • Provide counseling on mental health and behavioral health concerns, including depression and anxiety.
  • Maintain accurate, up-to-date patient records in compliance with healthcare regulations and organizational policies.
  • Participate in quality improvement initiatives to enhance patient care and optimize clinical outcomes.
  • Support patients and families with advance care planning, end-of-life care discussions, and referrals to palliative care services as appropriate.

Job Requirements

  • Active NP License in Oklahoma
  • Minimum of 2 years of experience as a Nurse Practitioner in an outpatient primary care setting, preferably working with seniors or geriatric populations.
  • Current BLS/ACLS certification
  • Driven to and comfortable with managing a primarily Medicare-aged, medically underserved patient population with comorbid conditions
  • Strong understanding of examination methodologies and diagnostics
  • Able to demonstrate interpersonal skills to be approachable and understandable to patients, families, and center team members
  • Confident in their ability to deliver high quality primary care, which includes being available to patients and addressing their medical and social needs
  • Passionate about their work and energized by being part of a fast growing organization
  • Comfortable traveling within the market to multiple centers within short distance of each other
