Job description

Position: Adult Day Health Center (RN) - Roxbury, MA

Overview: Provide primary nursing services to participants at the Adult Day Health Center. Provide initial and semi-annual nursing assessments; develop and implement nursing plan of care; provide medication management, nursing treatments, episodic assessments, and nursing interventions at the PACE center. Collaborate with Primary Care to provide complete nursing services in a safe, secure, accessible environment with skill, dignity and respect.

Essential Duties and Responsibilities:

  • Complete initial and subsequent semi-annual nursing assessments.
  • Implement physician orders and document in electronic medical record.
  • Provide a nursing assessment for pre-enrollees (potential new participants) during visit to PACE center and report findings to interdisciplinary team during clinic meeting.
  • Develop and implement a nursing plan of care for each participant on case load; update semi-annually.
  • Responsible for developing and implementing a medication management plan for each participant -- Implement physician orders, ensure delivery from pharmacy, document in medical record, assure administration of medication, provide education and training to participants and/or caregivers when needed to enhance adherence to medication regimen; Collaborate with Home Care Nurse for continuity of care at participant's home, including ALF, group homes, and nursing facilities.
  • Responsible for coordinating medication deliveries with transportation supervisor and/or transportation representative in a timely manner.
  • Accountable for monthly review of medications that are administered and dispensed at the center; assure medications are not expired, contaminated or deteriorated. Document monthly audit on audit tool.
  • Provide nursing assessments and interventions per a daily schedule and per episodic events at ADHC.
  • Provide Skilled Nursing Visits as needed, in collaboration with Primary Care and Nurse Manager.
  • Administer immunization screenings and vaccinations per policy and procedure. Document, review, and track administration in electronic medical record.
  • Provide documentation of all nursing interventions in electronic medical record.
  • Establish and maintain a collaborative relationship with participants, caregivers, and health aides to enhance provision of quality health care.
  • Report, document, and track the incidence of infections and wounds. Participate in data analysis, development, and implementation of improvement initiatives. Evaluate outcomes.
  • Report and document falls. Complete nursing assessment within 24-48 hours and document using documentation tool in electronic medical record.
  • Participate in the training, supervision, performance, and competency evaluation of Medical Assistants and Health Aides in collaboration with Nurse Manager and Center Manager.
  • Provide clinical supervision, support, training, and resource for Medical Assistants and Health Aides at ADHC.
  • Identify and coordinate the implementation of clinical duties with Center Manager, Medical Assistants, and Health Aides. Provide oversight of direct care [see Medical Assistant and Health Aide job descriptions].
  • Oversee and review data input by Medical Assistants and Health Aides in electronic medical record.
  • Participate in all clinical meetings (morning meeting, team, updates, and staff) and designated family meetings.
  • Participate in the on-call program per rotating schedule.
  • Perform other responsibilities as assigned by Nurse Manager.

Qualifications and Requirements:

  • Ability to work within interdisciplinary team environment
  • Good verbal communication and writing skills in English
  • Licensed Registered Nurse in Massachusetts
  • Baccalaureate Degree
  • Minimum 1 year experience working with geriatric population
  • Self-directed, organized, efficient, and flexible
