Job description

Social Workers needed!

Locations:

  • Clackamas & Hollywood (Portland Area) - 40 hours
  • Raines/Newberg (West of Portland) - 40 hours
  • Albany/Lebanon - 32 hours
  • Scholls Ferry/Noble Woods (Portland & Hillsboro) - 40 hours
  • The Dalles - 20 Hours

Provides psychosocial services to patients treated by the facility including in-center and home dialysis patients (if applicable) utilizing accepted methods of practice. Works with the health care team to promote positive adjustment, rehabilitation and improved quality of life for our patients. In collaboration with the interdisciplinary team, informs, educates and supports staff in understanding the emotional, psychological and behavioral impact of Chronic Kidney Disease on the patient and family to ensure quality care of our patients.

Day-In-The-Life

  • Social Worker has a case load typically between 130-140 patients
  • They have 2-3 rounds (shifts) each day where they treat patients for 4 hours at a time.
  • This includes meeting with patients and checking in on any concerns/needs, screenings and intakes, survey completions, and providing case management.
  • There are detailed documentation requirements for social workers and quality management goals to aim for with patients.
  • Some examples of case management services they could provide are making sure patients have access to transportation to clinic, working through logistics for getting proper medication, any financial barriers for patients
  • If patients are eligible and seeking a transplant - the social worker will monitor the process and act as a liaison with the transplant center
  • Educate patients on dialysis and adjusting to the process
  • Collaborating with team of physicians and dietitians as advocate for the patient

3-5 Must Have Skills/Qualifications

  • Masters in Social Worker
  • Clinical Social Work Associate (CSWA) through OR state

Schedule/Shift

  • Monday-Friday 8am-5pm (times vary)

Contract Length:

1040 Hours. Opportunity to convert to permanent hire after satisfactory completion of contract.

Pay:

$24.04 - $38.94 an hour (Dependant on experience)
