Social Work Care Manager

Job Description:

1. Must evaluate the potential client as a total person and identify the functional

limitations, which impede independent living.

2. Conducting in-depth assessments, reassessments covering psychosocial,

rehabilitation, and environmental concerns

3. Referring clients to special consultants to provide detailed evaluations and

recommendations to improve the client's functional level

4. Developing the care plan, implementing the services and conducting follow up

and monitoring of the plan

5. Identifying and developing support systems for the client
6. Consulting with the Health Practitioner
7. Case recording and reporting

Caseload:

15-18 in person home visits per month

Multi Senior Services Program - 65 and older

Qualifications:

• Bachelor's in Social Services Related field (Social Work, Psychology, or Sociology).
• Prior experience in any of the following: Case management, documentation, referral sources, DME (durable medical equipment), IHSS (in home support services), working with the elderly, etc.
• Great communication skills
• Computer Skills
• Must be Bilingual Spanish
• Driving - Drivers License and Auto Insurance
• Willing to drive heavily 2 days a week

Schedule:

Monday-Friday 8-5pm
