RN Case Manager - Hybrid

Medix is looking to hire a Registered Nurse for a Hybrid Case Manager position in Everett, WA.

Responsibilities:

• The RN Case Manager will be a part of the post-hospitalization team at the Clinic and will be assigned to patients.
• Oversee a caseload of 30 geriatric patients.
• Provide post discharge support after short and long-term hospitalization providing case management services.
• Reach out to the assigned patients, build rapport, conduct initial assessment of their needs and provide appropriate resources and guidance.
• Provide patient education and conduct motivational interviews to encourage patients reach and maintain their healthcare goals.

• Equipment will be provided to support WFH

Requirements / Experience

• RN License in WA State
• Prior case management experience in an outpatient setting (preferably internal medicine) and hospital transitions

Preferred Qualifications:

• Passion for working with elderly patients
• Nurse autonomy to make decisions on patient care
• Strong communication skills to effectively communicate with patients and families
• Being able to provide patient education and "motivational interviewing" (motivating the patients to change their behavioral and lifestyle to meet their healthcare goals)
• Confidence and ability for public speaking to present patient cases to the care team

Schedule:

• Monday- Friday 7:30 -4:00 pm
• Hybrid working model
• 1 committed day of the week on Clinic site
• 4 days of the week working remotely (WFH)

Pay range:

$40-50/hr BOE
