Clinical Assistant

Position Summary:

The Clinical Assistant assists with patient care including staffing with home care agency, facilitating communication between the intake nurses and pharmacy with home care agency, referral sources, and physician offices, and coordinating
documentation to ensure reimbursement for nursing visits is obtained. Clinical assistant also assists with gathering needed PHI including lab results and documenting appropriate communication in the patient record timely and with accuracy.

MAJOR RESPONSIBILITIES

Obtains patient referral data as quickly as possible.
Documents initial referral information.
Tracks required forms (AOB, ABN).
Facilitate pharmacy obtaining lab results and SNV notes.
Facilitate communication between intake nurse/pharmacy team and HHA for new and established patients.
Assist the intake nurse and pharmacy team with obtaining clinical information as requested.
Assist intake nurse and pharmacy team in communication with MD office.
Forwards all patient documentation to clinical staff in appropriate location and reimbursement staff.
Prepares and organizes new patient clinical chart.
Assists nurses and pharmacists in phone communication.
Staffs patients with HHA and communicates promptly and effectively.
Ensures proper documentation is present in the chart for reimbursement of SNV.
Assists in effective communication between the patient and the company, to include documentation of
suggestions, complaints, and requests
